Deeply regretted by his loving wife Margaret Forde, sons Stuart & Alexis, daughters Sarah Ellen & Rhoda, daughters-in-law Sherley & Jennine, son-in-law Anthony, grandson Kemi Jack, sisters Jane Leahy & Therese Murphy, brother-in-law Tom Murphy, sister-in-law Chrissie Daly, nieces, nephews, relatives, friends & neighbours.

Removal from his residence on Tuesday morning for Mass at 12 noon in St. Patrick's Church, New Quay, followed by burial in Corcomroe Abbey.

Due to government advice regarding public gatherings, a private family funeral will take place. A memorial service for Jack will take place at a later date.
