It was a pleasure to meet Jacob and your family and to work with you briefly, I will always remember the fun and love in your home, Jake's smile, wicked sense of humour and his strength and bravery, and that of the whole family. Jake, can't believe the angels have flown down to collect you but I'm sure they have taken you to a lovely place where you can now rest. You were a great pleasure to work with and support as well as having a bright sense of humour. You were such a fighter all the way to the end. You will be sorely missed. R.I.P and fly high Jake. Xxx

Absolutely devastated to learn of this news. Jake inspired me many times never to give up on a challenge and to keep going. I will treasure the memories of meeting him at his party back in Kent many years ago, such an infectious cheeky smile. I’m thinking of you all and hope you can draw some comfort from knowing he touched so many peoples’ lives. Sending you all our heartfelt condolences x

I will always remember Jake’s huge smile - especially when he’d ‘deliberately’ come out of his wheelchair again! IEveryone came running in to tell me I needed to help him back in his chair but by the time I got out to the field, he’d got himself back in. Never a scratch on him.
